PAC :
PAC (Pre-anaesthesia Check-up Clinic) is done prior to administration of any anaesthesia. The patients are assessed for administration of anaesthesia and surgery with respect to their:-
- Body reserve and can it withstand stress of surgery and anaesthesia medications
- Are they in optimum condition for the proposed procedure
- Feasibility of various techniques of anaesthesia
- Risks involved and any special needs etc.
The anaesthesia procedure is explained to the patient and pre-operative orders regarding fasting requirements and medications are also given to the patient. Patients are asked to bring all previous medical records including medications taken, investigations done in the past and are suggested to inform the doctor about any other illness. During PAC, the optimization of body of the patient is assessed for anaesthesia to be administered so that the risks involved remain the minimum possible. During the Pre-anaesthesia Check ups, at times patients are asked to take treatment and get some investigations done to optimize their comorbid condition and to know the latest status. It is compulsory that all adult patients who are going under anaesthesia must be empty stomach at least 8-10 hours prior to the surgery.
Operation Theatres :
The operation theatres are equipped with anaesthesia work stations and facilities for intraoperative monitoring of EKG, blood pressure (both non-invasive and invasive), temperature, oxygen saturation, anaesthesia gas monitoring, respiratory gas monitoring, depth of anaesthesia monitoring (BIS/entropy) and neuromuscular function monitoring. The operation theatres are also equipped with devices for management of difficult airway including fibreoptic bronchoscopes, video laryngoscopes and a variety of supraglottic airway devices.
ICU :
The Department of Anaesthesiology, MAMC has a 15 bedded Level 3 ICU. This is a designated ward of the hospital which is specially staffed and equipped to provide observation, care and treatment to patients with actual or potential life threatening illnesses, injuries or complications from which recovery is possible. The ICU provides special expertise and facilities for the multisystem life support for an indefinite period and utilises the skills of anaesthesiologists, medical, nursing and other staff trained and experienced in the management of these problems. It can provide mechanical ventilation, invasive cardiovascular monitoring for an indefinite period along with continuous monitoring and support of other organ systems of the body.As ICU deals with critically ill patients high level of sterility is maintained in this area along with restricted movement of general population. In our hospital ICU staff is also required to provide services outside the ICU such as emergency response and outreach services.
PAIN CLINIC :
The Department of Anaesthesiology is running the pain clinic from 2-4 pm from Monday to Friday in the PAC clinic, OPD block. Pain clinic treats patients suffering from chronic pain due to various diseases which include inflammatory disorders, chronic post operative pain, malignancy pain and neuropathic pain.
The modalities for pain relief are decided by the anaesthesiologists. These include pharmacological pain relief by drugs administered orally, intravenously, through trans-dermal patches or nerve blocks. Several non-pharmacological techniques are also used, such as accupuncture, laser therapy and transcutaneuous electrical nerve stimulation (TENS). The modality used depends upon the type of pain, site of pain and the underlying disease responsible for pain.
The department teaches and trains post-graduate students with 10 M.D. and 07 DA new admissions every year. The department conducts lectures, seminars, difficult case presentations and tutorials for the PG students which are moderated by faculty members. The department periodically conducts CME on Recent Advances in Obstetric Anaesthesia and workshops on Flexible Fiberoptic guided Intubation. The department regularly contributes in conduct of BLS and ACLS training courses for MAMC students. The esteemed faculty of the department is on the panel of various prestigious bodies like UPSC, MCI, NBE etc. The department has many national and international publications to its credit.
